Starting a new form
Go to the ‘Forms’ menu in the left-hand navigation and click ‘New Form’
This will open a blank form builder page, where you can enter all your required form fields and content.
Alternatively, if you already have a form which would be easier to ‘duplicate’ and edit to create a new form. Go to ‘Forms’ in the navigation, select the 3 dots menu beside the form you wish to copy and select duplicate. This will open a new form containing all the content of the form you selected.
You can now amend, add and remove content to create your new form.
Form intro section
Form name: Enter a name for your form (you don’t need to add the word ‘form’ at the end, as this is automatically appended in the template that goes to the client)
Form Intro: Enter an introduction, which is a short description to inform the client what the form is about.
Estimated duration: Enter an estimated time you think it would take a client to complete the form; this improves conversion rates.
This is what the content above would look like when sent to the client.
Automatically save completed forms to PMS
Beneath the form intro section is the option to select for completed forms to be automatically saved to your PMS and a .pdf attachment when the client submits them. (Only applicable where the PMS supports this)
Some PMS support the capability to save to either a client record or a patient record as a .pdf, where applicable, this can be selected from the drop-down.
Pages
Pages: The rest of the form is created as separate ‘pages’. Each page has a Page title and a Page intro. These appear at the top of the page when clients view the form.
Add further pages using ‘Add page’
Questions
Questions: Your questions will appear within the pages; you may choose how many questions you wish to display per page.
The question content types available are shown below:
Short text - One-line text entry
Paragraph - large text entry box
Email - one-line entry with basic email format validation
Date - date picker
Dropdown - select one answer from multiple options in a drop-down
Multiple choice - select one answer from multiple options in a list of radio buttons 🔘
Single checkbox - single select checkbox
Multiple selection checkboxes - multiple select checkboxes
Agree to terms - single select checkbox to agree to terms and conditions
Hidden (Clinic view only) - information that the client doesn’t need to see on the form, but is relevant for the clinic on the form. You can set this to prompt the sender to enter before sending, e.g. a note on the account stat,e or have it auto-filled from the PMS, e.g. client ID
Once you have chosen a content type, you will need to complete the following:
Question - This is the question that will appear to the client
Description - If the question needs more context, enter more information here (optional)
Placeholder text - A brief hint or example text which displays inside the answer field that guides users on what to enter, this disappears when the client starts typing.
Default - Pre-filled information or pre-selected option that appears automatically, helping clients complete the form easily and quickly.
For question content types that have options, e.g. multiple select checkboxes, you will see additional fields to complete.
Options - You will need to enter each option you wish the client to view and select from.
Show/hide value - This is an advanced option. As default, the entry input into the options label field will be auto copied across to the value field. This is to be used in scenarios where the clinic wishes to have what is displayed to the client in a form different to what is displayed on the clinic's view of the form once completed. e.g. client views user-friendly wording, but the clinic views a system ID which is only relevant to the clinic for internal use. In this case, the ‘label’ content will display to the client on the form, and the ‘value’ content will be what shows to the clinic in the completed form.
Further question settings:
Prefill this field - Enter a pre-filled answer or select to pre-fill the answer using PMS data. The options available to pre-fill using PMS data are as follows:
Client ID
Client name
Client postcode
Client phone number
Client email
Practice name
Patient name
Patient ID
Patient species
Patient breed
Patient age
Patient sex
Client address
Allow the clinic to change value before sending - Allows the clinic to specify an answer to display in the form on a per-client basis when sending. When the clinic sends the form to a client, they will be prompted with a modal screen prior to sending, where they can enter what they would like to appear in this field for that specific client. This can be used for scenarios where a clinic has a standard form to send to the client but may wish to specify specific information applicable to the client/patient, i.e. procedure name.
Add more question fields by selecting the ‘Add field’ button
Make this field read-only - Client will not be able to edit this field. This option is available when selecting ‘Prefill this field’ or ‘Allow the clinic to change value before sending’
This field is required - Makes the field mandatory for the client to complete.
Content fields
Within each form page, you can have ‘content’ field(s) which allows the setup of content to appear to the user within the form. A full content editor is available to allow you to add text, links and formatting.
